Self-Management

Spring House Medical Centre aims to work in partnership with you to help you to maintain your own health and manage any health conditions. You can expect us to help you take control of your health by giving you information and discussing treatment options with you. This will help you make informed decisions about improving or maintaining your own health. This self-management may involve medications and therapies. It can also mean making changes in lifestyle, such as eating better, reducing stress, or increasing exercise. We will expect you to take an active role in managing your health and wellbeing.

Self-management involves three different kinds of tasks.

  • You have to care for your body
  • You have to adapt your activities to your condition
  • You must deal with the hard emotions that illness brings.

Self-management support refers to what health care providers can do to help you in self-managing. Providers can support you by offering information, resources, and supportive care. This often includes listening well and helping you discover for yourself what you want to do.
