Requests for letters for benefits, council or any other third party will only be completed with a formal request from the third party.
All requests from the third party will require the patients formal consent.
We will only add factual responses; we cannot give an opinion.
If we are unable to complete will issue a copy of your patient summary instead with your consent.
HOUSING APPLICATIONS
We do not provide a GP letter if you are applying for a council home for health reasons or overcrowding.
Information in support of an application based on health grounds should be supplied by the applicant using a form provided by the Housing Department (self-assessment). This will not require any input from the GP or practice.
If additional information is required the Housing Department Medical Officer will ask the GP if they need it, provided the patient has given written consent.
