HOUSING/BENEFIT LETTERS

We can only complete letters or written reports for benefits, the council, or any other third party after we receive a formal request directly from that organisation.
 
Your formal consent is required before we can share any information with a third party.
 
We can only provide factual, objective information from your medical record.
 
We are unable to provide opinions or speculative statements.
 
If we are unable to produce the requested letter, we can instead provide a copy of your Patient Summary.

 

HOUSING APPLICATIONS

We are unable to provide GP letters to support council housing applications based on health reasons or overcrowding.

Applicants should complete the Housing Department’s self‑assessment health form, which is designed to capture all information needed for applications made on medical grounds. This form does not require any input from your GP or the practice.

If the Housing Department requires further medical details, their Medical Officer will contact the GP directly—but only if you have provided written consent.

FIT TO RETURN TO WORK INFORMATION

Our GPs are unable to provide “Fit to Return to Work” notes or letters, as these are not part of NHS‑funded services.

Medical Report for Employers

If your employer needs information about your health or readiness to return to work, a GP can provide a medical report.

Please note:

  • Requests must come directly from the employer.
  • We can only proceed with the patient’s written consent.
  • As a private (non‑NHS) service, fees may apply.

Next Steps

If your employer requires a report, please ask them to contact the practice via letter, which should include your consent before any information is shared.
