NON NHS SERVICES
Some services we provide are not covered under our contract with the NHS and therefore attract private charges.
Examples include the following:
- Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
- Insurance claim forms
- Prescriptions for taking medication abroad
- Private sick notes
- Vaccination certificates
- To Whom it May Concern letters
The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
FIT NOTE
If you have been unable to work for more than 7 days, including weekends and bank holidays you might need a fit note.
If you have NOT seen a GP at the practice about the problem and we have no information from a medical professional you will need to request a GP appointment before a fit note will be issued. You can request a GP appointment via our TOTAL DIGITAL TRIAGE SERVICE
If you have already seen a GP about the problem or we have recieved a letter from the hospital about it, you can then request a fit note via our TOTAL DIGITAL TRIAGE SERVICE
If you have been unwell for less than 7 days, you do not need to see a GP for a Fit note.
However your employer may need you to complete a self-certification form (SC2) which is available from your employer or you can download it.

You do not need to see a GP to Self Certificate
Please note that the GP may not be able to supply a certifcate depending on the information you provide, if you are under the care of a hospital, your certificate may be issued by the hospital, rather than by the practice.
