Digital Triage Service
On Monday 2nd June, Spring House Surgery switched to a “TOTAL DIGITAL TRIAGE SERVICE” where all requests for appointments, medical certificates, plus admin enquiries are to be submitted online.
Why is the system changing?
To allow the practice to prioritise patients based on their clinical need and will ensure faster care, improved continuity and better outcomes for our patients.
How will the system work?
All patients requesting an appointment must submit an online consultation form from our website Homepage or using the NHS App which will then be assessed by our Clinical team.
We aim to respond to every submission by text or phone within 48 hours, excluding weekends.
What we need from you
To help us assess your request please provide as much information as possible to include the following:
- Describe the problem, (please focus on one for example: I have lower back pain)
- Is this a new or ongoing issue, how long has it been going on for
- Have you tried anything to help?
- Your availbility to attend an appointment for example: The days and times you can attend an appointment
You may be offered an appointment within a very short turnaround time so you will need to ensure you are contactable.
If further information or clarification is needed we will contact you.
What about patients who find it hard to use the Internet?
Please call our reception team on 01707-294354, who will be available to talk you through how to use the system.
Administrative queries
The online consultation system must also be used for your administration queries such as requesting a medical certificate, and any other administrative query you may have.
When is it open?
Online submissions for clinical queries will be available from 7:30am until 1:30pm Monday to Friday.
If you have a medically urgent concern, please call the practice where you will be advised of an appropriate course of action.
